Boxer Angels Rescue Evaluation

This is used AFTER the dog has been surrendered  to rescue or taken from a shelter.   We want  to know more about the dog’s temperament,
training & medical needs.  Good temperament is the single most important aspect of a pet dog.

This form needs to be filled out within the first 24 hours.  The first evaluation should be only the areas you are comfortable with. 
The second evaluation needs to be filled out after the dog has been in your foster home for 5 days.  This evaluation needs to be
COMPLETED entirely.

It is important to remember that supplying complete information on this form will help place YOUR foster dog in the very best
